The following form will be used to obtain information regarding your refinance. This form should be completed BY REFINANCE BORROWERS ONLY. Once the form is completed you will have the ability to save a copy of the form that you submit to Parast Law.

Borrower Names - full legal name.
Borrower(s) Social Security Numbers (Tax id for entities).
Borrower(s) Email Address.
Borrower(s) Contact Phone - at least 1 phone number for each borrower.
If borrower(s) will be attending the closing.
Mortgages, Liens, or Equity Line of Credit on the property - Lender Name and Loan/Account Numbers.
